Course Authorization Forms

Course Preauthorization

To receive course booking authorization, all potential practicum students must complete the placement form for the course they want to book.

If you do not meet the prerequisites for a non-practicum EDS course, use the Enrollment Authorization System (EASy) to request enrollment authorization.

Fall 2026 Placement Authorization Request Forms for EDS Practicum Courses

How to Book an EDS Practicum Course

Because EDS practicum courses place UC San Diego students in community settings, additional information is required before course booking authorization can be issued.

  • Complete the placement form for your course to receive authorization to book one of the following courses: EDS-038, EDS-039, 128A, 128B, 129A, 129B, 129C, 130, 131, 133, 135, 136, 137, 139, 140, 141, 145, or 146.
  • Students who previously took an EDS practicum course and did not earn a passing grade of C- or higher in both the primary practicum course and the required EDS 139 corequisite course are not eligible to book.
  • Course booking authorization is typically provided within 1 to 2 business days.
  • After you are authorized, you will receive a confirmation email prompting you to log in to the Triton Student System (TSS) and book the course(s).
  • All practicum courses except EDS-038, EDS-039 and EDS-135 require the 2-credit corequisite course EDS-139.
  • You must book for the EDS 139 section taught by the same instructor as your practicum course.
  • A TB test is required for all in-person practicum work. Bring your test results to class on the first day.
  • TB testing can be completed through the Student Health Center.
  • EDS courses numbered 130 and higher (PAL): All new PAL practicum students must also complete the Volunteer Screening (Background Check) Form. Instructions and a link to the form can be found in the Placement Authorization Request form for each course. Questions about PAL course booking (Courses EDS-130 and higher)?
